1Passive with Worden
'Worden' + past participle for actions (focus on the process). Used in present and past.

2Passive with Zijn
'Zijn' + past participle for states (result of an action, not the action itself).

3Agent with Door
Use 'door' to introduce the agent (doer) in passive constructions.

door + agent

De beslissing werd genomen door de directeur.

Dutch uses 'er' for impersonal passive with intransitive verbs.
